Procedures

Insured persons need to undergo procedures for a change of address when moving or temporarily dispatched to a post away from family. Please correct the backside of your health insurance card (address field).

Procedures for a Change of Address

・If your address changes ・If the address of your dependents (family) changes (in the case of living separately)	Submit an Address Change Notification for an Insured Person Cross out your previous address with two lines on the backside of the health insurance card (address field), and then write in the new address on the next line.
If there is no more blank space in the address field	Submit “Application for Reissue of Health Insurance Card” (attach insurance card)
