Change of Name
If the insured person changes his or her name due to marriage or divorce, certain procedures must be followed.

Procedures for a Change of Name

Please notify your company immediately, attach an Eligibility Confirmation Document (only if issued) to the application form and submit it. For those who have an Eligibility Confirmation Document, the health insurance society will issue new Eligibility Confirmation Document with your new name based on information from your employer. *Even if you plan to continue using your maiden name at work, you will need to go through the process of changing your name.
*There is no need to attach your Individual Number Card.
