The Mercari Health Insurance Association provides health checkups to facilitate the early discovery of diseases and to enable everyone to check their personal health.
Be sure to get an annual health checkup!

*Please note that if you receive treatment at a non-contracted medical institution, there may be limits on the subsidy amount.

Secondary Examinations (Re-examinations)

Secondary examinations required due to examination results, re-testing, or more detailed screenings are considered medical care covered by insurance. Therefore, the insured person is responsible for paying 30% of the medical care costs as a general rule. Please bring a referral letter from the medical institution where you received the checkup and present your insurance card at the specialized medical institution counter before being examined.
Please be sure to check with the medical institution you will be visiting prior to undergoing an examination.

Scheduling Physical Examinations

The flow for scheduling a physical examination is as follows:

  1. Access the online Health Insurance Physical Examination Appointment Scheduling System. Click [List of Healthcare Providers], and then select the healthcare provider where you would like to have your physical examination.
  2. Schedule the examination.
    • Healthcare providers without appointment scheduling on the website
      (Appointments by phone)
      1. Call the healthcare provider and schedule the examination.
      2. Access the Physical Examination Appointment Scheduling System, and then click [Scheduled by Phone]. Enter the details of the appointment you made by phone.
    • Healthcare providers with a website to make an appointment
      (Online appointments)
      1. Access the Physical Examination Appointment Scheduling System, and then click [Schedule Online]. Request an appointment.
    *A user ID and password are required to log in.
  3. Receive the Health Checkup Approval Form and check the contents.
  4. Undergo the examination.
    Please be sure to bring the following on the day of the examination.
    • Health Checkup Approval Form
    • Sample containers, questionnaires, and any other necessary items sent by the healthcare provider
    • My Number Health Insurance Card, Eligibility Certificate, etc.
    *Individuals who do not qualify as an insured person or a dependent of an insured person on the day of the examination will be responsible for the full cost of the examination.

When undergoing a health checkup at a non-contracted medical institution

※Available only for residents in rural areas where there are no nearby medical facilities.
※Insurance-covered medical treatment is not eligible for subsidies.

Flow of the Health Checkup

  1. Please make an appointment with a health checkup facility on your own and undergo the examination.
  2. Please compile the following documents and send them by mail to the Health Insurance Association.

How to Submit the Health Checkup Expense Subsidy Application Form

Please either mail the Health Checkup Expense Subsidy Application Form, Health Checkup Report, Receipt, and Health Checkup Questionnaire to the Health Insurance Association or submit them through the electronic application system.

Changing/Canceling Appointments

  1. Call the healthcare provider where you scheduled the examination.
    Contact the healthcare provider where you scheduled the examination and get approval for the change or cancellation.

    Register the changes or cancellation of appointment.
    A new Health Checkup Approval Form with details of the changes registered will be sent to you by postal mail.
    (This notification is also sent to the healthcare provider.)
    *Be sure to contact Value HR Customer Service after receiving approval for the change or cancellation from the healthcare provider where you made the appointment.
